At the conclusion of the Panini NFL Player of the Day promotion, all shops are required to enter their shop winner into the National Sweepstakes and complete a Program Evaluation Form. Shops will receive an email with a link to the forms. or you can submit the paper forms included in your promotional kit via mail or fax.
